Get Credit for Your Blood Work

If you’ve been participating in the first round of our Be Well wellness program (January 14 through August 15, 2014), you hopefully know that blood work and other biometric screenings are part of making it to the first level of the program.

If you didn’t get to have your screening done in January during Health Scan Plus, and you still want to participate in this round of Be Well you have options.

  • You can have a free screening at any local Lab Corp location. Be sure to have this form completed before you walk into the lab facility. Your results will be transmitted to Aduro, the wellness program vendor, and they will put them into your Be Well account.

  • You can be screened at your doctor or get credit for a screening you’ve already had at your doctor’s office. To do so, give this form to your doctor’s office to complete and return. Again, Aduro will put your results into your Be Well account.
